Psychiatric Nurse Practioner

Apply evidence based practices and best practices critical time intervention and trauma informed care

Perform professional work that requires specialized to general knowledge of fields of advanced practice nursing, and a solid understanding, use, and application of concepts, theories, principles, practices, terminology, and applied bases of area of specialization, specifically psychiatry mental health, and addictions

Develop a nursing assessment tool to reflect the holistic needs of each client with consideration of his/her psychological, physiological, social, ethnic, cultural, educational and spiritual strengths and needs

Oversee the direct implementation of nursing care to ensure that it is carried out in accordance with the plan of nursing intervention and overall treatment/service plan and to ascertain if the needs of the client require modification of the plan

Conduct in-reach to hospitals and residences, engaging consumers and assessing for medical and psychiatric support needs in the community 

Participate in team meetings and provides nursing input on the development and implementation of treatment/service plans

Coordinate nursing care activities with other professional disciplines to ensure that the nursing plan is consistent with and integrated with the total treatment/service plan

Develop and implement that policies and procedures relating to medication management

Perform medication reconciliation, using technology as available

Provide health education/teaching for clients and their families
